If you apply for various certificates using the form below, we will send you a reply to the email address you entered with a QR code for the amount of the payment (issuance fee + shipping fee). Please save the code on your mobile phone and scan the QR code to make the payment. The certificate will be issued after we confirm your payment. If you are unable to read the QR code, the procedure will be the same as issuing a certificate by mail, so after filling out the form, please mail the amount of the payment (issuance fee + shipping fee) to the billing address below. The certificate will be issued after we receive the payment.*QR code payment (PayPay is only available in person)
