About certificate issuance

Issuance of various certificates

Various certificates can be issued in person at the counter, by mail, or via this website. When issuing a certificate in person or by mail, please print out the "Certificate Issuance Application" posted, fill in all the necessary information, and then complete the procedure in person at the counter or by mail.

Certificate issuance at the counter

Please fill out the "Certificate Request Form" or the school's designated form and apply at the counter. Certificates of enrollment and dormitory admission certificates will be issued on the spot if you wait about 20 minutes. In other cases, it will take a few days to be issued and will be returned at a later date. *Certificates for the Nursing Department Specialized Course (Specialized Course) will take a few days to be issued, so please apply well in advance.

Issuance of certificates by mail

Please fill out the application form, enclose the necessary information, postage for the certificate issuance fee, and a reply postage stamp, and mail it to the address listed below. 
<Reply postage> 1-3 copies: 140 yen / 4-9 copies: 180 yen 10 copies or more: 270 yen 
However, if you are able to collect it at the school office counter, this is not necessary. If you are unable to download the application form, please enclose a piece of paper with the following information written on it: ① Name ② Furigana ③ Name (maiden name) ④ Maiden name furigana ⑤ Romanization (only for those requesting English) ⑥ Date of birth ⑦ Gender ⑧ Graduation type (general course, nursing department, nursing major course, etc.) ⑨ Current address ⑩ Mobile or home phone number ⑪ Type of required document (which of the following is required: graduation certificate, transcript, or school report, and how many are needed)

Points to note

●About the time it takes to process certificates, etc. Depending on the certificate, it may take more than a week. Please apply well in advance.
● Regarding issuance fees, the issuance fees for each certificate are as follows. For certificates other than those listed below, please inquire by phone.

Issuance of academic transcripts (Nursing Department)

・Within 5 years after graduation (5 years): Issuance of academic transcript 
・6 years after graduation: Issuance of a certificate of credits (no grades listed, only the number of credits earned) 
[Note] For students of the 16th class and earlier of the Graduate School, the number of hours of study will be listed for each subject, rather than the number of credits.
